Billing:

· You may reach our billing office Tuesday through Thursday 10am—4pm

· If you have questions regarding a telephone charge on your statement, please see our after hour care policy.

· Co-pays are due at the time of service.  We accept cash, checks, Visa, MasterCard and Discover.  For your convenience, you may make a phone payment with your credit card by calling our billing office.

· At this time we are not accepting new patients with Community Health Plan.

· Be sure to check with your insurance carrier regarding your benefits.  Because, there are many insurance plans, we are unable to know every plan limitations that you may have.  Our billing department has put together some tips and suggestions regarding your insurance.

· During a well-child check, an issue or problem may arise that requires additional attention that may not be part of a routine well-child exam. You may be asked to make another appointment, or if possible, the doctor may address the issue in the same visit. There may be an additional, separate charge for this service.

 

 

Referrals:

 

· Please refer to your insurance benefits booklet to determine if you need a referral to see a specialist, or for special labs or imaging. Allow up to a week for the completion of the referral process.
