Billing:

· You may reach our billing office Monday through Friday from  10am—5 p.m.

· If you have questions regarding a telephone charge on your statement, please see our after hour care policy.

· Co-pays are due at the time of service.  We accept cash, checks, Visa, MasterCard and Discover.  For your convenience, you may make a phone payment with your credit card by calling our billing office.

· At this time, we are not accepting patients with Community Health Plan.

· Be sure to check with your insurance carrier regarding your benefits.  Because, there are many insurance plans, we are unable to know every plan’s limitation .  Our billing department has put together some tips and suggestions regarding your insurance.

· During a well-child check, an issue or problem may arise that requires additional attention that may not be part of a routine well-child exam. You may be asked to make another appointment, or if possible, the doctor may address the issue in the same visit. There may be a separate charge for this service.

 

Referrals:

 

· Please refer to your insurance benefits booklet to determine if you need a referral to see a specialist, or for special labs or imaging. Please allow up to a week for the completion of the referral process.
